SUMA Sports Event Waiver and Release Form

By signing below, I acknowledge and agree to the following:

1. Voluntary Participation: I (or my child/ward) am voluntarily participating in the SUMA Sports Community Event and understand that participation involves physical activities that may involve risk of injury.

2. Assumption of Risk: I understand and accept all risks associated with participation in sports and physical activities, including but not limited to falls, contact with other participants, equipment failure, and the effects of weather.

3. Medical Treatment: I authorize SUMA and its volunteers to provide or seek necessary medical attention in the event of injury or illness. I agree to be responsible for any medical costs incurred.

4. Release of Liability: I release, waive, and hold harmless the SUMA organization, its board, volunteers, event coordinators, and sponsors from any and all liability, claims, demands, and causes of action arising from my (or my child’s/ward’s) participation in the event.

5. Media Consent: I grant permission to SUMA to take photographs or video of me (or my child/ward) during the event for use in promotional materials, websites, and social media, without compensation.
